REGISTRATION INFORMATION:
You can use this form to register by mail for the Lay Lake Open Tournament.
Please see options below for full information.
REGISTER BY MAIL with a check or money order. Please fill in this
form and print it out. Then all you have to do is sign it and mail to
MARK'S OUTDOOR SPORTS, 1400 B Montgomery Hwy., Birmingham, AL 35216, along
with a check or money order made out to Mark's Outdoor Sports for the
amount of $125. Don't forget to include your e-mail address so that we
may send your confirmation.
NOTE: NO REFUNDS ON REGISTRATIONS
You must be present to win any prize, and you must enter the tournament to be eligible for any draw prizes.
I hereby waive and release all other contestants, the host, sponsors and tournament officials from all claims or injury and/or damage incurred in connection with this tournament. I further understand that Mark's Outdoor Sports reserves the right to refund this entrance fee if it chooses for any reason not to accept my application. I also understand that by submitting this information I agree to and abide by all the rules governing this event.
